Going on an expedition means experimenting, adding the compass, sometimes losing your way, getting lost and adjusting the final destination. But above all, trusting that the expedition will come to a good end.

Led by Patricia De Vriendt, Hanne Bossuyt, Lotte Vanneste and Marilyn Veraart in the PWO project expedition purposeful care, explore what purposeful care means. Together with participants from primary care, they look at how goal-oriented care can be implemented in their own setting and functioning.

Participants come together during a learning network and by means of coaching sessions, guided by Johanna Descamps and Christel Geerts, they work from a demand-driven approach. A learning network aims to bring care providers together to learn together about goal-oriented care. Not only personal goals, but also organisational goals are clarified in this way. Ultimately, individual care providers can bring goal-oriented care into the organisation and inspire colleagues. We are currently working together from the first-line zones of Midden-West-Vlaanderen and VVSG.

The aim of the researchers is to extract how this implementation is going. What does everyone need? And what might have been difficult? The findings will be elaborated both in a roadmap, including advice for those who want to get started with targeted care, and also in a more scientific way in which the collected data will be presented.
