Tailored support for diabetes patients
In the Diabetes and WELLbeing project (DWELL), eight partners from the UK, Belgium, the Netherlands and France are working together to enable individuals with type 2 diabetes to access tailored support. A 12-week programme was developed for persons with type 2 diabetes with various support options. The programme allows patients to manage their condition and increase their well-being.Â
The programme will be developed and implemented in the participating countries. The project should lead to an improvement in the health and well-being of persons with type 2 diabetes. It is estimated that the project will reduce healthcare costs by 30% per person. This amounts to €1.25 million per 1,000 patients per year.Â
More than €1.9 million of the European funding will be used for a cross-border partnership to develop approaches to tackle type 2 diabetes over four years.
What is included in the project?
- a cross-border training programme to benefit carers and patients;
- a 12-week programme to support patients in their lifestyle changes;
- a support network led by patients, to be maintained after the project;
- an evaluation of these programmes.
